- A circular medal. On the obverse, a contemporary crowned effigy of Her Majesty The Queen of Canada, facing right, circumscribed with the inscriptions “ELIZABETH II DEI GRATIA REGINA” and “CANADA”, separated by small maple leaves. On the reverse, a diamond-cut field, on which is superimposed a lozenge inscribed with the Royal Cypher, placed on four maples leaves, incorporating the dates “1952” and “2012” at the sides, and, in base, two scrolls bearing the words “VIVAT” and “REGINA” respectively. A 0.5 inch (12 mm) ring, which holds the ribbon, passes through a small hollow ball at the top of a claw joined to the top of the medal. The ribbon is 32 mm in width with a red stripe in the centre, 2 mm in width, on each side of which are white stripes, 3 mm in width, followed by red stripes, 10 mm in width and blue edge stripes, 2 mm in width. This ribbon is a variation of the 1953 Coronation Medal, 1977 Silver Jubilee Medal and 2002 Golden Jubilee Medal ribbons.

- The Queen's Diamond Jubilee Medal commemorates the 60th anniversary of the Accession of Her Majesty Queen Elizabeth the Second to the Throne as Queen of Canada. The medal design was unveiled on 3 February 2011 at Rideau Hall by the Governor General and the Prime Minister. The criteria for the medal were announced on the occasion of Victoria Day weekend in 2011 and on 6 December of the same year, the Governor General symbolically struck the first medal at the Royal Canadian Mint. This medal was awarded to Ken Verrell in 2012 for his public service which resulted in significant contributions to Elgin County and Canada.
